Last spring Hauppauge Public Schools partnered with FinalForms, an online forms and data management service for paperless/online registration. This year we are requiring that ALL parents of students involved in middle school clubs, intramurals, and athletics use FinalForms.
FinalForms allows you to complete and sign participation forms electronically for your children. Registration information is helpful for coaches and advisors to be able to communicate, take attendance, and more. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your children, saving you time.
You may review your data at any time to verify it is current. You will be required to sign your forms each time you want to register for a club/intramural or upcoming sports season, and/or anytime you need to make updates.
Fall sports registration is currently open for 7th and 8th grade students. The registration period will start 30 days prior to the start of the early winter, late winter, and spring sports seasons. For a list of athletic opportunities click here: 7th & 8th Grade Athletic Opportunities
