New York State requires that all students interested in participating in interscholastic athletics must provide a current Physical Examination, dated no later than 12 months from the start of the current season.

We are very excited to announce that Hauppauge Public Schools has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. The instructions and link to sign up can be found below.

Please be advised that all information must be sent to the school doctor after it is turned in to the Nurse's Office. This process may take a couple of days so please return your forms ASAP  to ensure that you are cleared to participate on the first day of practice. 

Final Forms will open for registration 1 month before the start of the season
